Closing the Gap

Overview

Townsville General Practice Network was successful in securing funding for three years through the Australian Better Health Initiative (ABHI) to undertake a project that will provide evidence of health outcomes in urban Aboriginal & Torres Strait Islanders populations.

It is widely recognised that there are major disparities in health status between Aboriginal and Torres Strait islander and non-Indigenous people and that chronic conditions is increasing due to a range of factors, some of which are largely preventable.

Objectives

This project aims to contribute to improving health outcomes for Indigenous people by promoting:

• culturally appropriate healthy lifestyle choices;

• supporting early detection of chronic conditions;

• culturally appropriate healthy lifestyle choices;

• supporting early detection of chronic conditions ;

• risk modification;

• encouraging active self management;

• improving integration and coordination of care.
