Enhanced Diabetes Management Program

This program (which started in 2007) supports practices to improve their management of diabetes by allowing regular auditing of their diabetic population. Education sessions are held in the practice for all members of the practice team, allowing them to review their own diabetes data and plan quality improvement activities. Information about the program is available in this manual. A powerpoint presentation is available explaining the evidence base for the program. Sample data is available here, and is typical of the data provided to the practice.

Australian Primary Care Collaboratives

TGPN is now (June 2008) a participant in this national program which targets diabetes, coronary heart disease and better access to General Practice. Practice staff participate in learning workshops and quality improvement cycles with support from a TGPN Program Officer. Further information about the program is available from the APCC website. Contact our Quality Improvement Program officer for more details.

Diabetes Type 2

The AGPN have a useful presentation on how to refer a Type 2 diabetic.

National Health Development Fund

In 2003, Townsville Division of General Practice and Queensland Health were involved in a National Health Development Fund — GP Integration Project which drew together private and public practitioners involved with caring for Type 2 Diabetic patients in the Townsville/Thuringowa area. As part of this project a patient hand held card was developed and this Diabetes Directory was produced to enable GPs to more appropriately refer their patients to Allied Health Practitioners and Specialised Services within the Townsville District.

  • Diabetes Australia Queensland website www.diabetesqld.org.au has information for patients and health professionals.
  • This site was launched in January 2008.
  • The National Centre for Chronic Disease Prevention and Health Promotion www.cdc.gov/diabetes This site can be contacted with questions. There is a list of frequently asked questions, news, information, publications and links to other agencies.

  • The International Diabetes Institute www.diabetes.com.au Is helpful with medical issues and includes the capacity for individuals to record and track blood glucose levels, blood pressure, pulse, cholesterol and weight online.

  • www.mydiabetes.com is a user friendly site that offers self care and tracking tools to help individuals take control of their disease management.

  • childrenwithdiabetes.com is an online community for kids, families and adults.

  • www.apma.org is a specialised site that concentrates on foot care for diabetics.

  • The Centre for Eye Research Australia site details Diabetic Retinopathy and useful information for diabetic patients eyesondiabetes.org.au
  • Type 2 Diabetes: Standard Care Pathway, a resource from General Practice Queensland supported by Queensland Health

Diabetes Patient Held Record

As part of the Diabetes GP Integration project in 2003 a template was developed to be filled in at the time of completion of the "Annual Cycle of Care". This template has an updated list of medications, room for Allied Health and GP comments as well as room for identification of all health care providers involved with the patient’s care. This update is easily available and can be done as often as necessary. The template can be used by itself or in conjunction with the Patient Hand Held Records which are available from TGPN. The template can be downloaded for MD or MSS.
