A Home Medicines Review (HMR) is a consumer-focused collaborative service involving the general practitioner and the local community pharmacist. The aim of a HMR is to optimise medication use and prevent medication-related problems through a team approach for patients living in the community. Medication related problems cause many unnecessary hospital admissions in Australia each year. There are many potential causes for medication related problems including poor compliance, confusion and the use of non-prescribed medication. The general practitioner and the pharmacist are both remunerated by Medicare and HIC respectively for their professional services. The process involves a referral from the consumer’s general practitioner to the consumer’s preferred community pharmacist. Any member of the healthcare team can ask the general practitioner to refer the patient for a HMR. A pharmacist interviews the consumer (preferably in the consumer’s home) to gather information about their medication. An accredited pharmacist assesses this information, writes a report and discusses the findings with the general practitioner. After discussion with the pharmacist, the general practitioner and the consumer agree on a medication management plan. The patient is central to the development of this plan.
