Patient Eligibility for the Reset Your Life Program

Eligible patients must be:

• aged 40 to 49 (inclusive)

and

• Scored 15 or greater as determined by the Australian type 2 diabetes risk assessment tool.

Patients with newly diagnosed or existing diabetes are not eligible for this item

The patient will be required to attend 7 sessions over a 6 to 8 month period to be held at the Townsville General Practice Network

A one off fee of $50 will be required to be paid to participate in the program (health care card or  concession card holders will not be required to pay this fee).

Reset Your Life Program Structure

Intensive phase of program

Phase 1 (fortnightly sessions)

·     SESSION 1 (2 HR) INTRODUCTION & WHAT IS DIABETES

·     SESSION 2 ( 3HR) PHYSICAL ACTIVITY & HEALTHY WEIGHT

·     SESSION 3 (1 ½ hr) INTRODUCTION TO HEALTHY EATING

·     SESSION 4 (1 ½ hr) PREVENTING CARDIOVASCULAR DISEASE

Phase 2 of program

·     SESSION (1 hr) (at 3 months) STAYING MOTIVATED & POSITIVE THINKING

·     SESSION 6 (1 hr) (at 4 months) STRESS MANAGEMENT

Feedback phase of program

·     SESSION 7 (1 hr) (at 6 months) FOLLOW UP

Use of The Australian Type 2 Diabetes Risk Assessment Tool 

• Completion of the Australian type 2 diabetes risk assessment tool is mandatory. The Australian type 2 diabetes risk assessment tool can be obtained from www.health.gov.au/epc.

• The tool can be completed either by the patient or with assistance with practice staff.

• Only patients with ‘high’ score(15+) results are eligible to attend a Type 2 Diabetes Risk Evaluation by their GP and, if appropriate, subsequent referral to a subsidised lifestyle modification program ‘Reset Your Life’.

 Role of the GP

1. Complete the 'The Australian Type 2 Diabetes Risk Assessment Tool'.

2. Undertake and arrange relevant investigations to exclude diabetes.

3. Assess the readiness of the patient to make lifestyle changes

4. Provide information and advice to the patient on lifestyle modification programs

5. Where appropriate arrange referral to The Townsville General Practice accredited lifestyle modification program - Reset your Life
