THE ORTHOPAEDIC TRIAGING TEAM: IMPROVING ACCESS TO SPECIALIST CLINICS

Referral Reform

Following a highly successful collaboration between the Townsville GP Network, Townsville Hospital Orthopaedic Specialists and TTH Referral Centre which resulted in the significant reduction in Orthopaedic waiting times. We have developed a new Referral process for referrals to Specialist Outpatient Clinics at The Townsville Hospital. The new process involves two steps. Step 1 is to fill out the Patient Information Form (which contains Patient demographic information) and Step 2 is to fill out the Clinic Specific Minimum Data Set form. At present we have only developed a Minimum Data Set form for Orthopaedics, Neurosurgery-Spinal and ENT for all other clinics we have a GENERIC Clinical Infomation form.

STEP 2

Clinic Specific Minimum Data Set (available via Website or contact 4725 8915 to have a copy emailed to you)

At present the ONLY Clinic Specific Forms available are listed below

Forms and resources

• Neurosurgery-Spinal
• ENT
• Generic Clinical Information Form for all other clinics
